Cancellation Policy
- As part of our service we offer a courtesy email confirmation at the time of booking your appointment/s and an email reminder approximately 24 hours before the scheduled appointment. As such 24 hours notice is required for cancellations. Cancellations within 24 hours of your scheduled appointment time will incur a cancellation fee of 50-100% of the scheduled appointment fee. This is at the discretion of Wholesome Physiotherapy.
- In the case that three consecutive appointments have been missed, unfortunately we are not able to make any further appointments for you.
- Please note that Wholesome Physiotherapy are unable to claim private health insurance claims with cancellations of appointments.
- Please have respect for all employees at Wholesome Physiotherapy with regards to these fees and other matters.
Payment Policy
- Wholesome Physiotherapy is a private billing medical practice. Payment can be made via credit/debit card. Wholesome Physiotherapy has HICAPS and EFTPOS facilities that allow you to claim your rebate immediately after your session.
- Payment of services is required at the time of appointment
- Clients will be charged for any additional reporting and resource development that may be required and will be billed accordingly.
- Full payment of completed reports and resources must be received before the report or resource can be released.
- Wholesome Physiotherapy reserves the right to change our fees at any time without prior notice.
Feedback and Complaints
- The staff at Wholesome Physiotherapy strive to provide you with the best possible service and highest level of care.
- If you would like to provide us with some feedback to improve our service or have a complaint, please let us know in writing. The feedback can be completely anonymous and can be forwarded to info@wholesomephysiotherapy.com.au
- Alternatively complaints can be sent to the Australian Health Practitioner Regulation Agency via their website https://www.ahpra.gov.au/
Privacy Policy
- Your medical record is a confidential document. It is the policy of Wholesome Physiotherapy to always maintain security of personal health information.
- This information is only available to necessary members of staff and other necessary users within the health system e.g. your GP medical practice, your private medical insurance company with your consent
- You have the right to request access to, and correction of, your personal information. If you wish to do so then you will need to put the request in writing via email and send it through to info@wholesomephysiotherapy.com
- At Wholesome Physiotherapy your personal and private information is important to us and we aim to comply with The Privacy Act 1988 and General Data Protection Regulations (GDPR).
Personal Information
Personal information applies to any information capable of identifying an individual. When you book an appointment with Wholesome Physiotherapy and complete the patient consent form you provide consent for our physiotherapists and practice staff to access and use your personal information so they can provide you with the best possible healthcare. Wholesome Physiotherapy may collect personal information about you through:
- The contact form on the website.
- Email and social media contact messaging.
- Reviews through google or other online sources.
- Any other communication that you send to Wholesome Physiotherapy
- The online booking system Cliniko
This data is processed by Wholesome Physiotherapy for communication and record keeping purposes only. Wholesome Physiotherapy does not, without your consent, supply your personal or privacy information to any third party.
